Priority: HHC is dedicated to the health, safety, and well-being of its residents, resident families, and staff.

Vision: Building upon HHC’s long tradition of exceptional service, the LTC Division is devoted to improving the lives of elderly and chronically infirm individuals in Indiana.

In 1995, HHC opened its first skilled nursing facility, Lockefield Village, on the campus of Wishard Hospital in Indianapolis. Since 2003, HHC has specialized in acquiring facilities that require capital improvements and sound management practices. HHC’s goal in operating such facilities is to substantially improve both the quality of care and quality of life for every resident.

In partnering with American Senior Communities (ASC), HHC manages a large portfolio of facilities across Indiana, encompassing 74 skilled nursing facilities with 7,885 licensed beds, five assisted living communities with 343 licensed beds, and 294 independent living units. HHC facilities consistently achieve CMS (Centers for Medicare & Medicaid Services) Five Star ratings that exceed state and national averages for Surveys and Quality Measures. HHC remains committed to continuous improvement, with a current focus on increasing Staffing scores across all locations.
