Oversight and Quality Review

The Health & Hospital Corporation of Marion County (HHC) maintains an internal Long Term Care (LTC) Division. The LTC Division is responsible for actively engaging with American Senior Communities (ASC) regarding HHC-owned skilled nursing and assisted living facilities.

Oversight and engagement is a daily, weekly, and monthly endeavor that includes review of all operational aspects of long term care facilities including participation in corporate compliance, quality assurance and performance improvement (QAPI), financial reviews, personnel, strategic planning, risk management and clinical services.

The internal LTC Quality Review Team conducts extensive clinical and administrative reviews on all of HHC’s facilities. Each facility is visited annually, and select facilities are visited a second time based on Indiana State Department of Health (ISDH) report card scores, CMS Five Star ratings, management turnover, and other factors that may warrant a follow-up visit.

The quality peer review covers many aspects of facility operations and clinical services, including an analysis of personnel files, training and clinical records, meal service observations, and healthcare-related treatments performed. Quality Review Teams include health facility administrators, registered nurses, social service professionals and administrative support staff with extensive experience in long term care and mental health settings.

ASC leadership at both the corporate and facility level meet with HHC to consider any recommended action plans or policy and procedure revisions.

HHC’s LTC Division also provides annual training in emergency preparedness and assistance in assuring facilities are provided the most up-to-date consultation. Emergency preparedness continues to be a primary goal for the LTC Division. HHC maintains a stakeholder agreement with MESH, a nationally recognized Emergency Coalition located in Indianapolis. This coalition serves all Indianapolis area hospitals and the Marion County Public Health Department. HHC’s LTC Division is the first and only long term care provider to join as a stakeholder.
